What To Do if MainStage 3 is Out of Tune or Transposed

In the MainStage 3 preferences, check the General tab, and at the top where it says Tuning, make sure it is set to 440 Hz:

Also, in the Audio tab, try changing the Sample Rate back and forth between 2 options (pressing Apply both times for older versions of MainStage 3):
